Step 3 - Home Study

Your application has been approved, and now it's time to begin the home study process. Your home study fee is now due. The payment link can be found on the  left-hand side of this page. 

 There are three distinct steps to your home study:

  • Paperwork
  • Training
  • Home visits

Paperwork

The paperwork for the home study can seem a little overwhelming, so we have broken it down into smaller steps, and we will guide you through the every step of the process. To help you stay organized, we will maintain a checklist, and you will receive a status update as documents have been received.

Initial Forms

There are a few things that need to be taken care of at the beginning of the home study process. First, families need to review our agency polices and procedures, as well as some resources from the state of Ohio. Those documents may be found on the Documents and Links page.   

After you have reviewed this information, you will need to upload these documents using the Document Submission form (left-hand side, bottom of this page) via our secure website:

  1. Drivers Licenses and Social Security Cards - these are needed to complete a check of the Ohio Central Registry of Child Abuse and Neglect. 
  2. Authorization to Release Information from another agency (only if you have had involvement with an agency prior to Spirit of Faith Adoptions)

State of Ohio Forms

Some of the documents that are required for your home study are forms created by the Ohio Department of Job and Family Services ("ODJFS"). Please scan and upload completed forms via the Document Submission form which is located on the left-hand side of this page.

* Please note that the Fire Inspection Report and Medical Statements are time sensitive. These forms need to be signed and dated within 6 months of the completion of the home study.  If for any reason you anticipate a delay in the home study process, we advise you to complete those specific forms last.

Supplemental Documentation

A number of supporting documents are necessary to verify information provided on some of the other forms.  These include:

  • Copies of birth certificates for each household member
  • Copy of marriage certificate
  • Copies of pay stubs for a two-month period (must be dated within 6 months of the home study completion date)
  • A copy of one month's utility bills, including each utility necessary to maintain the household
  • A copy of veterinary records for pets in the home (if applicable)

Criminal Background Checks

Both BCI and FBI reports are required for all applicants and household members 18 years of age and older.  Please have your prints taken electronically at a convenient location.  Locations can be found here: http://www.ohioattorneygeneral.gov/Business/Services-for-Business/WebCheck/Webcheck-Community-Listing.  Background checks should be run using code 2151 86.  Please request that the reports are mailed directly to Spirit of Faith Adoptions, 3315 Centennial Rd., Suite A-1, Sylvania, OH  43560.  Note: Household members who turn 18 years of age will need to have BCI and FBI background checks initiated within ten working days of the individual's eighteenth birthday.

Reference Forms

In addition to the spiritual reference obtained during the initial inquiry step, Spirit of Faith Adoptions is required to obtain four (4) personal references, as well as employment and childcare/volunteer references as applicable. The type of references required will be based on the information you list on your Application for Child Placement form. The online reference form can be accessed on the Documents and Links page. You are welcome to copy the link and email it to your references.

In some cases, the same person may be providing more than one type of references. The references form is divided into sections, so the person completing the form can respond to all the relevant sections at one time.

Additionally, if any applicant has seen a mental health therapist within the past 5 years, we require a letter, written by the therapist and printed on the therapist's letterhead. This letter may be submitted using the Upload a Document function.

Once we receive all required references, we will inform you and we will contact each reference to verify their information. You may want to alert them ahead of time. The contents of each reference is confidential and will be included in your home study document.

These additional forms will be completed online and can be accessed on the Documents and Links page: the Release of Information, the Information and Consent Form, Risk Acknowledgement Form, Receipt of Forms, and the Autobiography Information Form. You can also access a template for the fire escape plan to complete and upload using the Document Submission form.
